Community Leadership Application

To apply for the 2010-2011 Community Leadership program, please download and print the application or apply online.

If you choose to complete a printed application, it must include a $15 application fee payable to Leadership Lafayette. If you submit your application online, the $15 fee is waived. You may submit the printable application by one of the following methods:
